For your first visit to our pratice,
Most of the time, your doctor or dentist will refer you to us. We need the current address of every patient and information on their medical history and health status. It is also important for us to know which medications you take regularly. This information is recorded in the admission questionnaire. Please bring the following documents, if available:

You can download our forms here, which you can use to prepare for your visit to our practice. You can also have these sheets handed over to you when you visit us. We would be happy to discuss any questions with you personally in our practice.
You can obtain brief, general information on the most common surgeries from the following information sheets. Please note that all potential risks are recorded there.
We have written a short summary of our recommendations for behavior after a surgery for you in the following sheet. This will also be given to you after each operation.
The implant consultation sheet is intended for referring dentists who can use it to give their preferred implant position, implant system and prosthetic planning to the patient for a consultation in our practice. Please note that implant consultation is chargeable and is not covered by the statutory health insurance
